401(k)
Eligible employees can contribute up to 100% of their income into the 401(k) plan subject to the IRS limits established each year. Employees may enter the plan the 1st of the month following 90 days of consecutive full-time employment. Prefix does not require a waiting period for qualified rollovers. TopCareer Pathing

100% of employee paychecks are direct deposited into a variety of bank institutions of their choice. Employees may direct deposit to as many different accounts as they choose. TopEmployee Assistance Program (EAP)
Prefix offers its employees a free and confidential referral service to assist employees in handling work-life balance issues, financial planning, legal guidance, child rearing assistance, etc. Employees are eligible for this benefit as of their date of hire. TopFinancial Planning
Prefix feels that it is extremely important that employees plan for their futures. Therefore, eligible employees may work with the Company’s Financial Planner, through UBS, in order to seek sound financial advice for themselves and spouses. This service is provided for free when working directly with the Company’s representative. Employees may seek advice on various items including, but not limited to: how & what to invest in with regards to the Company’s 401(k), planning for retirement, planning for education, complete financial plans including living wills and trusts, attorney fees not included. Prefix offers its eligible employees a no cost life insurance and AD&D insurance policy at one times their annual salary. Amounts are maximized based on the insurance schedule. Employees are eligible the 1st of the month following 30 days of consecutive full-time employment. TopMedical Insurance
Prefix offers its eligible employees a comprehensive cafeteria-style benefit program. Employees may choose from three different medical insurance plans:- Plan A – A PPO through Blue Cross Blue Shield of Michigan – 100% in-network and 80% out-network. Dental & Vision provided only under Plan A.
- Plan B - A PPO through Blue Cross Blue Shield of Michigan – 80% in-network and 60% out-network.
- Plan C – A HMO through Blue Care Network – Most services are covered at 100% less applicable co-pays.
- Plan D – Opt Out Option – Prefix will reimburse employees not needing to take its health coverage a portion of the Plan C premium.
Prescription Drug Coverage
Prefix offers through each of its medical plans comprehensive prescription drug coverage. Employees are eligible the 1st of the month following 30 consecutive days of full time employment. TopReferral Program
Prefix values their employee input and feels strongly that the best employee is often the one that is referred into the organization by a current employee. Employees who meet the criteria and are in good standing with the organization may be eligible to receive a referral bonus for referring an excellent is expected Prefix Employee. TopShort Term Disability (STD)
Prefix offers its eligible employees a no cost Short Term Disability policy. Benefits include 66 2/3% of the employee’s weekly income up to $2,000.00 per week. Short Term Disability covers the 1st day of an accident or injury and day 8th for an illness. Employees are eligible the 1st of the month following 30 days of consecutive full-time employment. Prefix offers its eligible salaried employees 40 hours of sick/personal time each year. This time is advanced to employees after 90 days of consecutive full-time employment and is prorated upon termination. Employees may carry over up to 40 hours each year. Time is tracked via the employee’s anniversary date. TopSuccession Planning
Prefix is serious about its longevity in the marketplace and in identifying individuals to carry the Company into the future. As such Prefix’s Senior Management Team is constantly evaluating individuals for its succession plan and grooming those individuals for positions it has identified. TopTuition Reimbursement
Prefix encourages its employees to further themselves and their education. After 6 months of full time employment, eligible employees may receive reimbursement up to 50% of the cost of tuition for approved classes for a passing grade of “C” or better. Reimbursement is subject to management approval. TopVacation Time
Prefix offers its full-time eligible employees paid vacation time. Vacation benefits increase by the more seniority an individual earns with the Company. The maximum number of vacation days an employee may earn is 20 days. This time is advanced to employees and is prorated upon termination. Time is tracked via the employee’s anniversary date. Prefix offers a comprehensive vision plan to eligible employees who choose to elect Plan A. Employees are eligible the 1st of the month following 30 days of consecutive full-time employment. TopVoluntary Life & Accidental Death & Dismemberment Coverage
Prefix offers eligible employees an opportunity to purchase up to 5 times their annual salary in additional life insurance or accidental death and dismemberment insurance, over and beyond what Prefix provides its employees for free. The voluntary program is a post tax deduction taken the first payroll of each month. Employees are eligible the 1st of the month following 30 days of consecutive full-time employment.
